Enrollment Procedure

FOR OLD STUDENTS:

  1. Present your duly accomplished Student’s Clearance slip upon enrollment. (Please disregard this if you have already submitted to the Registrar prior to your Final exam).
  2. Present your valid University ID card to secure a print out of grades from your previous semester and or a checklist from the Registrar’s office. (Disregard this if you have submitted to the Registrar prior to your Final exam).
  3. Proceed to Window 5, present your ID to secure IAU Form No. 4 – Enrollment Form.
  4. Fill in the Enrollment Form with the subjects you wish to enroll for the coming semester. Pertinent information at the back of the form should also be filled up. PLEASE WRITE LEGIBLY.
  5. Present the checklist together with you filled up Enrollment Form the panel of evaluators (Administration office), for review of taken subjects, failed grades and verification of pre-requisite subjects and for approval of your Enrollment Form.
  6. Proceed to the Registrar’s office (Window 1 or 2) for encoding and assessment of fees.
  7. Pay at the Accounting Office (Window 3, 4 or 5).
  8. Proceed to Window 6 for the confirmation of enrollment and releasing of your study load.
  9. Enrolment is Complete.

FOR NEW STUDENTS:

  1. Proceed to the Information Table and ask for IAU Form 001–Personal and Background Form.  PLEASE WRITE LEGIBLY.
  2. Present the following documents in a long brown envelope, (write your name at the upper left corner)
  • Original and one(1) photocopy of Birth Certificate (NSO Authenticated)
  • Original and one(1) photocopy of Certificate of Good Moral Character
  • Original copy of Form 138 (High School Report Card)
  • Transcript of Records (TOR) from previous school.
  • Honorable Dismissal/Certificate of Transfer Credentials
  • Two (2) copies each of 1”x1” and 2”x2” color pictures (blue background)
  1. Proceed to the Treasurer/Cashier for payment of the Examination Fee (Php300.00).
  2. Proceed to the Guidance Office (2nd floor of the Administration Bldg. staircase at the back of the Treasurer’s office) for the Examination. (Submit the photocopy of your admission requirements).
  3. Upon obtaining the Examination results, proceed to the Administration office for an interview with the Department Head.
  4. Return the Examination and Interview Results to the Guidance Office.
  5. Submit all original documents to the Registrar’s Office.
  6. Secure IAU Form No. 4 – Enrollment Form at Window 5.
  7. Fill in the Enrollment Form with the subjects you wish to enroll for the coming semester. Pertinent information at the back of the form should also be filled up. PLEASE WRITE LEGIBLY.
  8. Present the checklist together with you filled up Enrollment Form the panel of evaluators (Administration office), for review of taken subjects, failed grades and verification of pre-requisite subjects and for approval of your Enrollment Form.
  9. Proceed to the Registrar’s office (Window 1 or 2) for encoding and assessment of fees.
  10. Pay at the Accounting Office (Window 3, 4 or 5).
  11. Proceed to Window 6 for the confirmation of enrollment and releasing of your study load.
  12. Enrolment is Complete.
